INTRODUCTION: In 1993, Cuba achieved leprosy elimination according to the World Health Organization's (WHO) indicator of less than one case per 10,000 population. Despite this achievement, detection of new cases occurs every year among all age groups including children. Detection of new cases in children reveals persistent transmission of the infection. OBJECTIVE: To describe the clinical and epidemiological features of leprosy in individuals younger than 15 years (childhood leprosy) reported to the Cuban National Leprosy Control Program (NLCP) between 2012 and 2019. METHODS: We conducted a retrospective descriptive study between 2012 and 2019 to assess the clinical and epidemiologic features of individuals under the age of 15 years with a confirmed diagnosis of leprosy reported to the NLCP. We reviewed the NLCP database and collected data to better define the total number of cases of leprosy in adults, children (younger than 15 years). We assessed socio-demographic variables (age, gender, and province of residence) as well as variables of clinical interest including operational classification and staging at diagnosis, bacillary index, grade of disability by WHO staging. Additionally, we evaluated epidemiological variables including passive versus active surveillance of cases, contact investigation focusing specifically in household transmission, and the degree of kinship as well as standing of the child within the focus of transmission when there were additional cases. RESULTS: We identified fifty children during the study period corresponding to 3% of the overall cases of leprosy comprising all age groups in Cuba. In the age group younger than 15 years, the majorities of cases was from the Granma province and most were between the ages of 10 and 14 years. Clinically, multibacillary/lepromatous forms were the most common type identified with positive bacillary index. The majority of children diagnosed with leprosy during our study period had a history of a relative with a confirmed diagnosis of leprosy. CONCLUSIONS: Detection of cases of leprosy in individuals younger than 15 years of age in Cuba demonstrates ongoing transmission of M. leprae in specific geographic hotspots. Its frequency in the early adolescence, the predominant clinical forms, and the mode of detection associated with sources of suspected familiar infection demonstrated that there is a need for further efforts by the NLCP to conduct active surveillance activities among affected communities to identify cases of leprosy earlier with the goal of preventing further household and community transmission.

BACKGROUND: Children are believed to be the most vulnerable group to leprosy. Childhood leprosy reflects disease transmission in the community as well as the efficiency of ongoing disease control programmes. In Cuba, leprosy is not a national health problem; however, new childhood leprosy cases are diagnosed every year. OBJECTIVE: We summarise the experience of Cuba on childhood leprosy control over the past two decades. RESULTS: Between 2000 and 2017, a total of 103 children in Cuba have been diagnosed with leprosy, showing that active transmission of cases remains in 13 of 15 provinces of Cuba. The majority of cases were multibacillary (66%), and 34% were paucibacillary cases. Clinically 60% of children have more than five lesions all over their body. Voluntary reporting was the principal method of case detection. The presence of familial and extrafamilial contact with leprosy cases may be a cause of concern, as it implies continuing transmission of the disease. Only four children had disabilities (one with grade 2 disabilities and three with grade 1 disabilities). A set of national investigations have been developed to intervene in a timely manner. Intervention strategies that combine clinical surveillance and laboratory test could be an option for early detection of childhood leprosy. CONCLUSIONS: Early detection of cases due to effective health education campaigns, regular and complete treatment with MDT, and contact tracing may be important in reducing the burden of leprosy in the community.

[RESUMEN]. En el presente artículo se identifican rasgos distintivos y esenciales en las transformaciones del sistema de salud cubano que han permitido la erradicación y disminución de las tasas de incidencia de algunas enfermedades transmisibles a niveles inferiores a 0,1 por 100 000 habitantes. Los resultados obtenidos son consecuencia de la importancia otorgada a la prevención y control de estas enfermedades, así como a los riesgos y daños potenciales. La estructura y funcionamiento del subsistema de higiene y epidemiología y sus interrelaciones con el resto del sistema a partir de los diferentes modelos de prestación de servicios han constituido escenarios de integración permanentes para la toma de decisiones. Se eliminaron enfermedades como la poliomielitis, paludismo, difteria, tosferina, rubéola, parotiditis, meningitis posparotiditis, sarampión, fiebre amarilla, cólera, formas graves de la tuberculosis, rabia humana transmitida por caninos, leishmaniasis, enfermedad de Chagas, la transmisión vertical del virus de inmunodeficiencia humana, sífilis congénita y formas clínicas como el tétanos neonatal y el síndrome de rubéola congénita. Se hace énfasis en algunas enfermedades transmisibles y, en especial, en la respuesta social desarrollada contra la tuberculosis, la lepra, el sida y las enfermedades transmitidas por vectores. Se demuestra que el contexto sanitario actual revela aún desafíos para la sostenibilidad de los logros alcanzados en el país. Garantizar el mantenimiento de la cobertura universal con acceso de la población cubana a los servicios de salud será siempre un principio de la salud pública cubana.


[ABSTRACT]. This article presents distinctive and essential features in the transformations of the Cuban health system that have allowed the eradication and reduction of the incidence rates of some communicable diseases at levels lower than 0.1 per 100 000 inhabitants. The results obtained are a consequence of the importance given to the prevention and control, as well as to the risks and potential damages, of these diseases. The structure and functioning of the hygiene and epidemiology subsystem and its interrelations with the rest of the system, based on the different models of service provision, have been permanent integration scenarios for decision making. Diseases such as poliomyelitis, malaria, diphtheria, whooping cough, rubella, mumps, post-mumps meningitis, measles, yellow fever, cholera, severe forms of tuberculosis, human rabies transmitted by canines, leishmaniasis, Chagas disease, vertical transmission of HIV, congenital syphilis and clinical forms such as neonatal tetanus and congenital rubella syndrome were eliminated. Some communicable diseases are analyzed in more detail and, in particular, the social response developed against tuberculosis, leprosy, AIDS and vector-borne diseases. However, the current health context presents challenges for the sustainability of the achievements made in the country. Assuring the maintenance of universal coverage with access of the Cuban population to health services will always be a principle of Cuban public health.

No presente artigo se identificam características específicas e essenciais nas transformações do sistema de saúde cubano que permitiram a erradicação e redução das taxas de incidência de algumas doenças transmissíveis a níveis inferiores a 0,1 por 100 000 habitantes. Os resultados obtidos são uma conseqüência da importância dada à prevenção e controle dessas doenças, bem como aos riscos e danos potenciais. A estrutura e o funcionamento do subsistema de higiene e epidemiologia e suas inter-relações com o resto do sistema, com base nos diferentes modelos de prestação de serviços, constituíram cenários de integração permanente para a tomada de decisões. Foram eliminadas doenças como a poliomielite, malária, difteria, coqueluche, rubéola, parotidite, meningite pós-parotidite, sarampo, febre amarela, cólera, formas graves de tuberculose, raiva humana transmitida por cães, leishmaniose, doença de Chagas, a transmissão vertical do vírus da imunodeficiência humana, sífilis congênita e formas clínicas como o tétano neonatal e a síndrome da rubéola congênita. É dada ênfase a algumas doenças transmissíveis e, em particular, à resposta social desenvolvida contra a tuberculose, a hanseniasis, a AIDS e as doenças transmitidas por vetores. Mostra-se que o atual contexto de saúde revela desafios para a sustentabilidade das realizações no país. Garantir a manutenção da cobertura universal com acesso da população cubana aos serviços de saúde sempre será um princípio da saúde pública cubana.

Introducción: La Lepra es una enfermedad infecciosa causada por el Mycobacterium leprae. Los patrones dermatoglíficos de pacientes cubanos con lepra lepromatosa mostraron indicios probatorios de que existe predisposición genética para el desarrollo de esta enfermedad, que sugiere la búsqueda de la asociación con polimorfismos moleculares, de mayor precisión. Entre estos, unos de los más estudiados son el T352C del gen del receptor de la vitamina D y el A16974C del gen IL12p40, cuya utilidad relativa depende de la población. Objetivo: Determinar si existe asociación entre la presencia de los polimorfismos T352C y A16974C con la lepra lepromatosa en pacientes cubanos. Material y Métodos: Se realizó un estudio observacional, analítico, de tipo caso-control de asociación genética donde se estudiaron pacientes con lepra lepromatosa y controles. Fueron identificados los genotipos relacionados con los polimorfismos T352C y A16974C en cada grupo. La prueba Chi-cuadrado de Pearson fue utilizada para determinar si los controles se hallaban en equilibrio de Hardy Weinberg, así como si existía relación entre los polimorfismos y la presencia de la enfermedad. Resultados: Los pacientes estudiados fueron 32 para el polimorfismo T352C y 44 para el A16974C. Los controles fueron 64 y 44, respectivamente; estos se hallaron en equilibrio Hardy-Weinberg. No se detectó asociaciónentre los polimorfismos A16974C y T352C con la lepra lepromatosa. Conclusiones: Los polimorfismos T352C y A16974C no son útiles como factor de riesgo predisponente en el grupo de pacientes cubanos con lepra lepromatosa estudiados(AU)


Introduction: Leprosy is an infectious disease caused by Mycobacterium leprae. Dermatoglyphic patterns of Cuban patients with lepromatose leprosy showed evidential signs of the existence of genetic predisposition to the development of this disease, which suggests a search for the association with molecular polymorphisms of higher degree of accuracy. Among them, some of the most studied are the T352C vitamin D receptor gene and the A16974Cof the IL12p40 gene, which relative usefulness depends on the population. Objective: To determine whether there is an association between the T352Cand A16974C polymorphisms with lepromatose leprosy in Cuban patients. Material and methods: An observational analytical case-control type genetic association study was conducted where patients with lepromatose leprosy and controls were studied. Genotypes related to T352Cand A16974C polymorphisms were identified in each group. Pearson´s chi square test was used to determine whether the controls were in Hardy-Weinberg equilibrium, and also whether there was a relation between polymorphisms and the presence of diseases. Results: There were 32 patients under study for T352C polymorphism, and 42 for A16974C. The controls were 64 and 44, respectively; and these were in Hardy-Weinberg equilibrium. No association between T352Cand A16974C polymorphisms with lepromatose leprosy was detected. Conclusions: T352Cand A16974C polymorphisms are not useful as a predisposing risk factor in the group of Cuban patients with lepromatose leprosy studied(AU)

La Lepra es una enfermedad infecciosa causada por el Mycobacterium leprae. Sin embargo, hay personas que bajo los mismos factores ambientales presentan la enfermedad, mientras que otros son resistentes. Se conoce que los dermatoglifos se alteran cuando existen factores genéticos que afectan el desarrollo fetal. Relacionar los hallazgos dermatoglíficos con la presencia de lepra lepromatosa constituyó el objetivo de este trabajo. Se realizó un estudio transversal, analítico de tipo caso-control no pareado en 30 individuos con diagnóstico de lepra lepromatosa, entre septiembre del 2012 a junio del 2014. Como controles se utilizaron los descritos por Borbolla y colaboradores para la población cubana normal, realizándose comparaciones según mano y sexo. Se observó diferencias con lo clásicamente descrito en los controles cubanos. Estas incluyeron: ausencia de arcos en el dedo cuatro de la mano izquierda y tres de la mano derecha en los hombres. Presencia de bucle radial en dedo tres de la mano izquierda en las mujeres y del trirradio axial t en ambas manos en hombres. En uno y otro sexo se observó un elevado porciento de pliegue simiesco. En nuestros casos se puede sugerir que los dermatoglifos pueden constituir una herramienta útil para el estudio de la predisposición a esta enfermedad…(AU)
